Transport, Logistics & Delivery Edition · Module 6

Breakdowns, Damage, Missing Items & Incident Escalation

Transport and logistics work involves movement.

Movement creates the possibility that something may go wrong.

Examples may include:

  • vehicle breakdown
  • delivery-bike malfunction
  • damaged goods
  • missing parcel
  • lost company equipment
  • failed delivery
  • customer complaint
  • route incident
  • damaged vehicle
  • disputed handover
  • package tampering concern
  • unexpected operational disruption

An incident does not automatically mean someone was careless.

But how employees respond after the incident matters.

A manageable problem can become much worse when someone:

  • hides it
  • continues without reporting
  • invents a story
  • blames another person without evidence
  • makes an unauthorized repair
  • changes the record
  • abandons goods
  • waits until the end of the day to mention something serious

Strong incident response asks:

WHAT IS HAPPENING NOW?

IS ANYONE OR ANYTHING AT IMMEDIATE RISK?

WHO NEEDS TO KNOW?

WHAT FACTS DO I HAVE?

WHAT SHOULD I NOT TOUCH OR CHANGE?

WHAT IS THE NEXT AUTHORIZED STEP?

BY THE END OF THIS MODULE, YOU SHOULD BE ABLE TO:

  • respond appropriately when work is disrupted
  • distinguish immediate response from later investigation
  • report vehicle or equipment problems through the proper route
  • respond responsibly to damaged customer goods
  • handle missing items without premature accusations
  • preserve relevant information after an incident
  • communicate facts without speculation
  • understand when escalation is urgent
  • avoid hiding incidents or making unauthorized corrections
  • participate responsibly in post-incident follow-up

Core Behavior

Protect what needs protecting, report what you know, and escalate serious incidents through the appropriate route.

TrueConduct Principle

Do not create a second problem while trying to hide the first.

Remember

An incident may be unavoidable.

Dishonesty after the incident is a separate choice.
